Steps

Cast The Balrog from the command zone by paying .
Activate Phyrexian Altar by sacrificing The Balrog, adding .
Choose to return The Balrog to the command zone from your graveyard.
When The Balrog dies, it and Blight Mound trigger.
Resolve the Balrog trigger, destroying any artifact and/or creature an opponent controls.
Resolve the Blight Mound trigger, creating a 1/1 Pest creature token.
Activate Phyrexian Altar by sacrificing the Pest, adding .
When the Pest dies, it triggers, causing you to gain one life.
Repeat.
