How does this combo work?

2 | When Cytoplast Manipulator dies, its persist ability triggers, returning it from your graveyard to the battlefield with a +1/+1 counter on it due to its graft ability. |
3 | When Cytoplast Manipulator enters, Blessed Sanctuary triggers, creating a creature token. |
4 | When the creature token enters, Cytoplast Manipulator triggers, moving a +1/+1 counter from it to the creature token. Cytoplast Manipulator dies as a state-based action due to having 0 toughness. |
5 | Repeat from step 2. |
