Activated Sleeper must be in hand. Karmic Guide must be in the graveyard. Altar of Dementia must be on the battlefield. Mana needed:

Steps

1. Cast Activated Sleeper by paying , choosing to have it enter the battlefield as a copy of Karmic Guide.
2. Activated Sleeper enters the battlefield, targeting Karmic Guide.
3. Holding priority, activate Altar of Dementia by sacrificing Activated Sleeper, causing target player to mill cards equal to Activated Sleeper's power.
4. Resolve the Activated Sleeper trigger, returning Karmic Guide from your graveyard to the battlefield.
5. Karmic Guide enters the battlefield, targeting Activated Sleeper.
6. Holding priority, activate Altar of Dementia by sacrificing Karmic Guide, causing target player to mill cards equal to Karmic Guide's power.
7. Resolve the Karmic Guide trigger, returning Activated Sleeper from your graveyard to the battlefield as a copy of Karmic Guide.
8. Repeat from step 2.
