Danitha, Benalia's Hope must be on the battlefield. Altar of Dementia must be on the battlefield. Fungal Fortitude must be on the battlefield (attached to Danitha).
Steps
  1. Activate Altar of Dementia by sacrificing Danitha.
  2. When Danitha dies, Fungal Fortitude triggers, returning Danitha from your graveyard to the battlefield tapped.
  3. When Danitha enters the battlefield, it triggers, returning Fungal Fortitude from your graveyard to the battlefield attached to Danitha.
  4. Resolve the Altar of Dementia ability, causing target player to mill cards equal to Danitha's power.
  5. Repeat.
